This privacy notice intends to serve as the basis on which any personal information and sensitive personal information that you provide will be processed, controlled and protected by CICT.

• What information do we collect?

  These are not normal times that we live in, with the rise of COVID-19 global pandemic it is our duty to observe the health of all CICT employees and its affiliates.

  The Health Checker App will be used to monitor the health status or medical condition of CICT employees as well as its affiliates during this ordeal. And for us to fulfill the objective of this program, we will be collecting your personal information and sensitive personal information.

  • The personal information that we may collect from you includes your: Employee’s Mobile Number
  • In order for us to determine if you have symptoms similar to COVID-19, we shall collect your sensitive personal information such as your health data or medical condition which you should record each time you use the Health Checker App. Through this application, we may be able to assess your current health status by requiring you to register manifestations of COVID-19 symptoms such as dry cough, fever, runny nose, sore throat, breathlessness, headache, body aches, sneezing, fatigue and diarrhea.
  • The Health Checker App when accessed via browser, it uses Cookies to record your browser’s version, operating system, date and time of access, MAC Address as well as your current location the moment you login to the Health Checker App. Cookies is a small piece of data sent from a website and stored on your computer while you are browsing.
  • We may also collect through this Health Checker App your previous and planned foreign or local travels, including your job function and day-to-day task deliverables.

  We adhere to the principle of proportionality and only collects your personal information and sensitive personal information that are necessary to fulfill the objective of this program. We will not collect any personal information and sensitive personal information that are irrelevant and does not serve any purpose.

• How we collect your information?

  To appropriately monitor your medical condition amid the COVID-19 pandemic, you will be asked to record your health condition in the Health Checker App. In order to use this application, you will be asked to provide your personal information (see Section 1.a) for verification and authentication. We ensure that your personal information and sensitive personal information is secured and protected in accordance with this Privacy Notice and in compliance with the Data Privacy Act of 2012. We will keep your personal information and sensitive personal information as long as necessary.

  Before you can use the Health Checker App, you will be asked to provide your mobile number. This will help CICT to authenticate you as a user and a legitimate employee of Fortis ICT Solutions Inc. and its affiliates.

• How we protect your information?

  By submitting your personal information and sensitive personal information, you agree to the collection, processing, retention, and storage of your personal information and sensitive personal information at our authorized and secured database location.

  All confidential information you provide to us is transmitted using Secure Socket Layer (SSL) encryption technology and may be stored on our secured servers. Where we have given you (or where you have chosen) a password which enables you to access the Health Checker App, it is your responsibility to protect it and keep it confidential.

  In addition to the steps that we take to protect your personal and sensitive data, you should take the following precautionary measures to help protect yourself against unauthorized disclosure or fraud:

  • choose a password you can remember but others will not guess, and if you do write it down, keep it somewhere safe and secure;
  • when you have finished any session on any of our websites, make sure that you prevent your details from being seen by any third parties or unauthorized persons, and if you have registered and logged in, remember to log off;
  • clear any ‘cache’ so there is no record of any transactions left on screen; and
  • clear your browser history so the session history is removed.

  We adhere to the Data Privacy Act of 2012 and its IRR and it is our obligation to ensure that proper security controls are in place and are effectively implemented through our information security and data privacy policies and procedures. We have enforced adequate organizational, physical and technical security measures to appropriately safeguard your personal and sensitive personal information.

• How we use your information?

  We will be using the personal information and sensitive personal information you provided us for the following purposes:

  • to help us monitor your health condition for the duration of this COVID-19 pandemic; and
  • provide us visibility on all CICT and its affiliates employees current condition and wellness, travel plans and whereabouts, including job deliverables and tasks.

  The personal information and sensitive personal information you provided us would be processed by a data visualization software called Tableau and only authorized CICT personnel will have access to this tool with a need-to-know basis to prevent misuse of information, and to form an aggregated data.

  The Health Checker App will also be used for contact tracing of employees who are identified persons under investigation (PUI) or proven positive of COVID-19. We encourage you to be transparent and truthful on recording your existing medical condition for this will help us take the necessary precautionary/remedial measures and prevent the spread of the virus, which may eventually compromise the health of your co-workers, frontliners and health workers.

• How we share your information?

  However, if you are a PUI and have been proven positive of COVID-19 symptoms, authorized personnel of CICT will notify you personally thru your registered email address. We may also seek for your consent, which is freely given, specific, and you indeed agree on sharing your personal data to the general public or relevant government agencies for the purpose of contact tracing, and for us to take proper corrective and precautionary measures. Our goal is to prevent the spread of the virus and protect our employees, your co-workers as well as your family members.

  The Department of Health may share or disclose your information subject to the limitations that the sharing is:

  • pursuant to a public function or a public service;
  • based on the constitutional or statutory mandate of the DOH and/or the other public authorities;
  • strictly following set protocols and processes;
  • ensuring the security of such shared information; and
  • upholding your rights as a data subject.

  We will impose strict security protocols when transmitting or transferring your personal information and sensitive personal information to government agencies.

  If you are COVID-19 positive, CICT may release the necessary internal communication notice to the whole organization without disclosing your identity.

• Rights and access to your information

  In compliance with the Data Privacy Act of 2012, we value your rights as a data subject.

  • Right to be informed – this Privacy Notice sets out the essential guidelines of collection, processing, storage, retention, transfer and disclosure of your personal information and sensitive personal information while taking into consideration the principle of proportionality, legitimate purpose and transparency.
  • Right to access – CICT is in possession of the information you provided us, and as a data subject you have do request a copy of the personal information and sensitive personal information you have submitted and gathered by us.
  • Right to rectify – if you suspect of found out that you information is outdated or inaccurate, you have the right as a data subject to modify your personal data or you may send a request to privacy@Fortisict.com so we make the necessary amendments.

  Since we are in the state of public health emergency, and the basis of processing of data is not consent, CICT as a private entity we follow the mandate of the Department of Health in response to the COVID-19 pandemic.
